• Auto-Renewable Membership: Your membership is set to automatically renew after the initial term on a month-to-month basis, as agreed upon, at the specified monthly cost until cancelled using the appropriate process outlined below.
• Cancellation Process: To cancel your membership, please follow the appropriate process starting with completing the form provided below. Written cancellation requests require 30 days' notice and become effective 30 days after the date received. Any payments due before the effective cancellation date will be charged as scheduled.
• Initial Term Cancellation: During the initial membership term, you may only cancel your membership if (a) you permanently relocate your residence beyond a 25-mile radius from Juicy Boost location, or (b) a licensed Medical Doctor (M.D.) certifies that you are unable to receive Juicy Boost services. For all cancellation requests, written proof of relocation (e.g., Mortgage or Rental agreement, utility bill, car insurance) or submission of the Medical Doctor's note is mandatory. Upon approval of your written cancellation request during the initial term, you will no longer be required to make any future membership dues payments.
